About the role Grampians Health Ballarat Commonwealth Home Support Programme (CHSP) and  Home and Community Care Program for Younger People (HACC PYP) provides Allied Health, Nursing and Social Support Groups for people to assist them to remain living at home and in their community.  This new role is an exciting opportunity to join our multidisciplinary CHSP and HACC PYP teams.   Key Responsibilities First point of contact for CHSP and HACC PYP inquiries Develop and maintain efficient processes and systems  Data support and management Adminstration support for CHSP and HACC PYP team Skills and experience Administration and customer service experience is essential for this role, with experience working within health and/or community services preferred.
