You will be seeing clients in community that are referred through the following government funded models: Home Care Packages (HCP) • Short Term Restorative Care (STRC) • Commonwealth Home Support Programme (CHSP) We also accept privately funded referrals. 2. Caseload- Above 65 years of age for community home care referrals 3. You will be seeing a range of clients with various health conditions ranging from musculoskeletal, neurological and cardiorespiratory type referrals. Most commonly they would require equipment prescription, home modifications, Activities of Daily Living (ADLs) assessment and retraining. We welcome Occupational Therapists interested in enhancing their skills in the Home Care space, as our team includes experienced leaders and supervisors ready to provide support & training in this area. Skills and Qualifications: Occupational Therapy Bachelors’ Degree or equivalent as approved by AHPRA – Full Registration Drivers’ license and reliable vehicle Professional indemnity insurance, or willingness to obtain. Valid NDIS Worker Screening Clearance, or willingness to obtain. 2023 Flu Vaccine Certificate, or willingness to obtain. COVID vaccine certificate, or willingness to obtain. Excellent time management and organisational skills Ability to work independently in a remote team environment. Experience and skills in prescription of equipment and home modifications preferred.
