This pivotal role entails being the initial point of contact for our clients, playing a crucial part in guiding them through the service intake process and ensuring they receive the necessary support.

Responsibilities :

  • Initial Client Assessment : Conduct comprehensive assessments of potential clients’ needs and eligibility for services.
  • Information and Referral : Provide accurate and up-to-date information about available services, and refer clients to appropriate resources and programs.
  • Documentation and Record Keeping : Maintain detailed records of interactions, assessments, and referrals to ensure continuity of care and compliance with policies.
  • Coordination with Service Providers : Work closely with therapists, social workers, and other professionals to coordinate services and develop personalized care plans.
  • Client Follow-up : Regularly follow up with clients to assess their satisfaction with services and make adjustments as necessary.
  • Advocacy and Support : Advocate on behalf of clients to ensure they receive the services and support they are entitled to.
  • Community Outreach : Engage in outreach activities to raise awareness about the organization and identify individuals who could benefit from our services.

Qualifications and Skills

  • Education : Bachelor's degree in Social Work, Psychology, Rehabilitation, Special Education, or related field preferred.
  • Experience : At least two years of experience in social services, disability services, or a similar role.
  • Communication Skills : Exceptional interpersonal and communication skills, with the ability to effectively communicate with clients, families, and service providers.
  • Organizational Skills : Strong organizational and time management skills, with the ability to manage multiple tasks and prioritize effectively.
  • Empathy and Sensitivity : Demonstrated empathy and sensitivity towards the needs and concerns of individuals with disabilities.
  • Problem-Solving Skills : Ability to analyze situations, identify challenges, and develop practical solutions.
  • Knowledge of Disabilities : A solid understanding of various disabilities and the challenges they present is essential.
  • Computer Literacy : Proficiency in using computer systems, databases, and office software to maintain records and manage information.

Key Responsibilities:

  • Referrals : Act as the first point of contact for referrals from various sources, ensuring a smooth and efficient process for potential clients to access our services.
