Your new role

The RMH NDIS pathways improvement lead is responsible for developing and implementing standardised pathways for care to reduce the length of time that patients spend in hospital after they are medically ready for discharge. This work will contribute to improvements on other RMH access KPI’s that includes NEAT and AV offload.

Reporting to the General Manager HFACC this role will work in partnership with the RMH Services to proactively manage allocated project activities including implementation of approved initiatives. These initiatives will be aimed at achieving the vision of the Access and Flow strategy utilising a data informed and collaborative approach.

About You :

  • Allied Health clinician. Registered with AHPRA or eligible for membership of professional association with Tertiary qualifications in either; health, business or management.
  • Significant experience working with NDIS participants
  • Experience advocating for patients in a complex health and community service system
  • Demonstrated experience of managing change in a complex environment.
  • At least 7 years clinical experience as a practicing Allied Health professional
