About the RoleThe Care Coordinator will possess demonstrated experience within the aged care sector, with a specific focus on Home Care Package (HCP) management.  Candidates should possess exceptional communication skills, enabling them to effectively liaise with potential aged care Home Care Package clients, supporting their goals to remain living independently in their own homes.  This role will be covering our client base in the northern suburbs, including Bundoora, Preston and Reservoir.  This is a hybrid role, with responsibilities split between any of our Melbourne offices and the flexibility of working from home.Key ResponsibilitiesUndertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsCollaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice LeadsDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & Experience  Degree or equivalent in allied health, nursing, mental health or a related field as well as previous complex case management experience.Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for individuals and their carers, including people with culturally and linguistically diverse backgrounds.Demonstrated experience in facilitating and maintain partnerships with service providers and health professionals.Demonstrated experience in the use of brokerage services in a community setting.
