Joining KinCare may be exactly what you are looking for!As a Community Physiotherapist with KinCare, you will visit elderly individuals in their homes to deliver services and programs aimed at helping them maintain their independence and feel confident engaging in the activities they love. You will be responsible for assisting customers in maintaining their strength, balance and mobility, while also providing support to manage chronic health conditions including cardiovascular disease, musculoskeletal disorders, diabetes and dementia. Conducting assessments, supporting care plans, and implementing intervention strategies will be essential, enabling customers to remain at home for as long as they wish.Working closely with a multidisciplinary care team comprising Nurses, other Allied Health staff, Case Managers and Support Workers, you will contribute to supporting customers' overall health and well-being. Operating autonomously, you will communicate regularly with families, other clinicians, and the KinCare team to plan, implement, and evaluate tailored physiotherapy services for your customers.In this permanent, part-time role, we offer three days per week (you choose the days!) but they must be consistent each week. Our office is conveniently located in Notting Hill, but you will have the flexibility to work from home when not visiting customers. You will work with customers in the South/East suburbs of Melbourne, including St Kilda, Ringwood, Pakenham, Frankston and more.About you: Degree or Postgraduate Qualification in Physiotherapy Current national registration with AHPRA Community experience highly desirable, along with knowledge and experience in delivering the Otago Exercise Programme or similar Experience working with people experiencing chronic health conditions, including cardiovascular disease, musculoskeletal disorders, diabetes, and dementia Permanent part time availability (3 days per week) Full Driver’s licence and a reliable vehicle with insurance Ability to work autonomously and within a multidisciplinary team Genuine interest in working in the community and with the elderly (previous experience an advantage) Strong communication and interpersonal skills About KinCare:KinCare is one of Australia’s leading in-home care providers.
